Keto can work for a man over 40, but not because it burns fat faster. It works when cutting out most carbs helps you eat fewer calories without counting. In controlled trials, low-carb and low-fat diets produce similar fat loss over a year. Keto suits men who like firm rules and don’t miss bread and beer much. It’s a poor fit for men who train hard, eat out often, or have high LDL cholesterol.
What keto actually is
A ketogenic diet keeps carbohydrate low enough that your body shifts to burning fat and producing ketones for fuel. Researchers often define it as a maximum of about 50 g of carbohydrate a day, with non-ketogenic low-carb diets sitting at about 50 to 150 g (Aragon et al., 2017).
In food terms, 50 g of carbs is roughly one medium bagel, or two bananas, for the whole day. A typical keto day is built from meat, fish, eggs, cheese, nuts, oils, avocado and non-starchy vegetables.
Does keto burn more fat?
Not in the trials that have measured it carefully:
- The ISSN’s review concluded that a wide range of diets, from low-fat to ketogenic, can be similarly effective for body composition, and that fat loss is driven by a sustained calorie deficit (Aragon et al., 2017).
- DIETFITS randomized 609 adults to healthy low-carb or healthy low-fat for 12 months. Weight loss was 6.0 kg versus 5.3 kg, a difference that wasn’t statistically significant (Gardner et al., 2018).
- An NIH inpatient study compared an animal-based ketogenic diet with a plant-based, low-fat diet, letting people eat as much as they wanted. People ate about 690 fewer calories a day on the low-fat diet, contrary to what the “carbs drive overeating” theory predicts (Hall et al., 2021).
None of that means keto fails. It means keto works the way other diets work: by reducing what you eat. For some men, removing an entire food group makes that easier.
Pros and cons for a man over 40
| Pros | Cons |
|---|---|
| Clear rules; no counting for some men | Hard to keep up at work lunches, family dinners and social events |
| Cuts out most ultra-processed snack foods | Beer and most cocktails are out |
| Some men report fewer cravings | Hard training sessions can feel flat |
| Fast early drop on the scale (mostly water and stored carbohydrate) | That early drop can reverse quickly when carbs come back |
| Easy to pair with high protein | Low in fibre unless you plan vegetables, nuts and seeds carefully |
| LDL cholesterol rises substantially in some people |
The cholesterol question
Keto diets heavy in red meat, butter and cheese are high in saturated fat. Canada’s food guide recommends limiting saturated fat and choosing foods with unsaturated fats instead. Responses vary a lot between people. In a large survey of people on a very low-carb, all-meat diet, median reported LDL cholesterol was markedly elevated (Lennerz et al., 2021); that’s a more extreme diet than standard keto, but it shows the direction of risk.
If you try keto, build it on olive oil, nuts, avocado and fish more than butter and processed meat, and ask your doctor for a lipid panel before you start and about three months in. If you already have high cholesterol, read the cholesterol guide first.
Keto and training after 40
Strength training relies partly on stored carbohydrate. Some men find their first few weeks on keto noticeably harder in the gym, especially for higher-rep or high-intensity work. If muscle gain is your main goal, a diet with more carbohydrate around training usually serves you better.
Who should avoid keto or ask first
Keto isn’t right for everyone. Talk to your doctor before starting if you take insulin or other diabetes medication, since blood sugar can drop quickly; if you take blood pressure medication; or if you have kidney disease, high LDL cholesterol, gallbladder problems or a history of pancreatitis. Men with a history of disordered eating should also avoid rigid food rules.
If you want to try it
A sensible approach for a man over 40:
- Get bloodwork first (lipids and blood sugar) and tell your doctor.
- Keep protein high, around 1.6 g per kg of body weight. Keto isn’t a high-fat free-for-all.
- Eat plenty of non-starchy vegetables for fibre.
- Set a trial period of 8 to 12 weeks and judge it on weekly weight averages, waist size, lifts and how you feel.
- Plan your exit. Decide in advance how you’ll reintroduce carbs so the weight doesn’t come straight back.
Is keto worth it for you?
Keto is a legitimate tool, not a metabolic shortcut. If it makes eating less feel easy and you can live with the restrictions, it can work. If you miss carbs, train hard or eat out a lot, a less restrictive diet will probably get you further. Compare it with the alternatives in the best diet guide, or the Mediterranean diet if heart health is a priority. If you’d like help choosing an approach and adjusting it monthly from your results, that’s part of monthly 1:1 coaching.
Questions people also ask
How many carbs a day is keto?
Most definitions set a maximum of about 50 g of carbohydrate a day, often lower. Above that, most people won't stay in ketosis. A non-ketogenic low-carb diet usually means about 50 to 150 g a day.
Is keto bad for your cholesterol?
It varies. Some men see LDL cholesterol rise substantially on high-saturated-fat versions of keto. If you try it, get your lipids checked by your doctor before you start and again after about three months.
Can I build muscle on keto?
You can maintain and build some muscle with enough protein and a calorie surplus, but hard, high-volume training often feels worse with very low carbs. Men focused on muscle gain usually do better with more carbohydrate.
What's the 'keto flu'?
Headaches, fatigue and irritability in the first week or so as your body adjusts to very low carbohydrate. It usually passes. Drinking enough fluid and not cutting salt too hard in that first week can help.
